PROVIDER REGISTRATION
To register your organization for access please complete the Addendum to Provider Agreement on the website and enter the requested information. You will need to print and sign the form and submit it using the confirmation number you are provided. Once your Addendum has been reviewed and approved you will receive an email letting you know the Addendum has been approved.
This form only needs to be completed once. Once the Addendum has been completed and approved you can register as a user and provide the below information to users for them to obtain access.
USER REGISTRATION
Once your Addendum to Provider Agreement is approved, you can register to obtain claim information, payment information, and participant eligibility by clicking the New User Agreement for eCMS button and entering the requested information. You will need to print and sign the form and submit it using the confirmation number they are provided. Once you submit the signed user agreement the application will be reviewed and once approved, you will receive a user name and temporary password.
